The position is an integral part of the primary care team, responsible for helping members improve their health by providing them with tools and motivation to make healthier choices. The Health Coach is the member's first point of contact when it comes to health and wellness matters. The incumbent reaches out to members when they are identified on reports by members' preferred method of contact. The position fields calls, emails, and texts on the Wellframe dashboard from members. The Health Coach hosts health fairs and group educational sessions and serves as an advocate for patients in all areas related to health and wellness.


Essential Responsibilities/Accountabilities:


  • Empowers participants to make life long behavior changes or modifications, for improved health outcomes, by establishing goals and action steps.

  • Helps the participants identify ways they can overcome barriers, so they can achieve their goals

  • Conducts onsite health-related wellness seminars.

  • Provides accurate & appropriate education and guidance, within scope of education

  • Personalizes the coaching sessions for each person.

  • Partners with Care Managers to drive quality in relation to QAAR, STARS.

  • Completes detailed records in the Health system that address the health concerns for each individual; complete documentation per protocol timeframe.

  • Updates the monthly coaching reports within one week of receiving it and/or by the due date provided.

  • Makes reminder calls, emails, scheduling, or rescheduling clients for needed appointments.

  • Offers Wellframe and meet monthly Wellframe engagement goals.

  • Travels to clients location or perform telephonic calls as needed for individual coaching; HRAs or report deliveries.

  • Complete report deliveries if assistance is needed, both on-site and by phone.

  • Performs other tasks requested, such as: office jobs, projects, host health fairs etc., and other functions as assigned by management.

  • Consistently demonstrates high standards of integrity by supporting the Lifetime Healthcare Companies mission and values, adhering to the Corporate Code of Conduct and leading to the Lifetime Way values and beliefs.

  • Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.

  • Regular and reliable attendance is expected and required.

  • Performs other functions as assigned by management.


Minimum Qualifications:


  • ACE Health Coach Certification is required. In lieu of certification, will accept LPN, Medical Assistant, Health and Wellness Coach experience with a minimum of 5 years working in a medical insurance company or medical care setting required.

  • Two years of experience working in the wellness industry is preferred.

  • Ability to influence participants by being a good example for wellness.

  • Experience in motivational interviewing or Behavior Economics

  • Stay on top of current information for health recommendations.

  • Know basic nutrition, health, and fitness information, including knowledge in tobacco cessation, stress management and health risk reduction.

  • Be able to work on a team, but also work independently; and be self-motivated and resourceful with proven abilities to multi-task and operate successfully under deadlines and time pressures.

  • Be able to work with different personalities; education levels; different stages of change; and be flexible and accommodate their needs.

  • Enjoys a fast paced and challenging work environment.

  • Be honest, trust worthy and reliable.

  • Maintain complete confidentiality and a high degree of accuracy for all work performed.

  • Be innovative and open to new ideas, approaches and information.

  • Motivated, positive individual with high energy level.

  • Be a good communicator with superb verbal and written skills, as well as being a good listener.

  • Have a positive can do attitude that understands participants obstacles and appreciates the participants successes.
