Overview

The Population Health Management Strategist is accountable for managing, coordination and end to end oversight of the required cross-functional, inter-disciplinary team structure for Population Health Management initiatives. Works with the Chief Medical Officer and other senior Health Care Services leaders to develop targeted Population Health Management strategy.... The deliverables and outcomes will align with the organization.........s strategic plan, regulatory requirements and accreditation bodies. Advances Independent Health disease management, care management, physician care and primary care to achieve our collective/strategic IH population health goals and fill gaps in care. This position will work collaboratively with Informatics and IT to identify necessary infrastructure, data and work streams required to achieve provider and member 360 views necessary for effective PHM.

Qualifications

  • Bachelor.........s degree required. An additional four (4) years......... experience working with population health/public health project management may be considered lieu of a degree.
  • Formal clinical education from an accredited clinical program in a heath related field such as RN, OT, PT or Pharm D with licensure in New York State required.
  • Eight (8) years of health care, managed care and/or integrated delivery expertise experience with in-depth working knowledge of population health/disease management and clinical operations required.
  • Four (4) years of leadership experience required.
  • Strong working knowledge of health care delivery in context of population health strategies, programs and initiatives.
  • Experience and demonstrated success in strategic planning and analysis with ability to accurately interpret / predict /assess /validate population health/disease management trends/cohorts with operational expertise to understand and successfully guide the clinical operations processes.
  • Demonstrated ability to lead clinical based initiatives to a successful outcome.
  • Ability to effectively work, influence and build relationships with all levels in the organization.
  • Excellent written and verbal communications skills with emphasis on diplomacy, negotiation and persuasiveness and interaction with multiple levels of management.

Key Accountabilities

  • Accountable as the subject matter expert/content expert in Population Health Strategic Initiatives/Program Development, with focus on select disease states/conditions/use cases in constructing/deploying an end to end programmatic solution/deliverable across an integrated healthcare delivery system for all IH membership (all lines of business).
  • Program design/redesign in a matrix organizational structure/in concert with the Health Care Services Leadership team to include case management, disease management and quality improvement, pharmacy, etc. as to ensure high quality care and with improved member/provider outcomes measures.
  • Collaborate in matrix organization/reporting structure with Case Management, Pharmacy staff, and Medical Directors for specific programs that support population goals.
  • Advance disease management, care management, physician education, and primary care to achieve our collective/strategic IH population health goals
  • Seamless execution of a disease management (i.e....... use case......) based on actionable data analytics with ability/expertise to track, trend, measure and monitor program success /programmatic outcomes.
  • Participate in Population and Disease Management collaborative organizations as appropriate.
  • Ongoing assessment of program effectiveness in concert with population health goals/strategies and make the necessary changes /adjustments so as to assure programmatic success.
  • Provide oversight for coordination and provision of Population Health Management program activities and ensure compliance with regulatory requirements and NCQA standards related to Population Health Management.
  • Participate/support IH Population and Disease Management collaborative organizations, provider networks, integrated delivery systems, etc.
  • Coordinate data from BAI, Behavioral Health, Utilization/Case Management, Pharmacy and QI departments in assessment of the population, identification of members for eligible programs and measuring outcomes of the programs.
  • Data reporting collection/ provision of documentation of strategy and programs related to NCQA compliance, NYSDOH, CMS.
  • Contribute and work with all IH departments as a thought leader and subject matter expert to advance IH in areas of population health strategy with IH as a population market leader, and as an organization that is viewed/seen as competitive differentiator in the population health/disease management arena.
