The OB/GYN department is looking for a Care Coordinator to provide complex and diversified support to the Care Team (provider, medical assistant, nurse, etc.) by coordinating care for patients on the daily schedule; this support will require advanced administrative skills and knowledge. Work from a team approach, and in continuous partnership with patients and providers to promote: timely access to needed care, comprehension and continuity of care, and the enhancement of patient well-being.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Serve as an effective communication link between patient and clinic staff by gathering information from patients.
  • Independently and proactively identify patients in the practice by running panel reports for specific diagnosis and preventive care needs.
  • Evaluate and analyze patient records. Communicates with patient directly to discuss preventative care needs and refers patients with medical concerns to appropriate clinical staff.
  • Review provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.
  • Work with clinic team on developing standards, implementation, and maintenance of programs in chronic disease management for patients.

EDUCATION AND EXPERIENCE:

  • Graduate of an accredited Medical Assistant program.
  • Current Washington State Medical Assistant-Certified credential required. Also acceptable is active MA-Interim credential - must convert to active MA-Certified credential prior to expiration of Interim credential.
  • Also considered is pending MA-C / MA-I which must be converted to active MA-C or MA-I within six months of hire date. Will be required to obtain MA-Registered and work at MA-R level and scope, must convert to MA-I/MA-C within six months of hire.
  • Certification from national certifying entity (AAMA, NCCT, AMT, NHA, etc) as required for MA-Certified credential.
  • Current healthcare provider BLS.
  • At least two years relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.
