GENERAL SUMMARY:
The Manager of Case Management works under the direction of the Director of Case Management to plan, organize, implement, and evaluate the medical management/case management program at Florida Hospital. This includes capacity and length of stay (LOS) management, discharge planning, and social work for the hospital in accordance with regulatory standards, and in cooperation with the Director, the Medical Director, and all operational hospital departments. The Manager works with the Director, case management staff and the interdisciplinary health care team to facilitate campus-specific capacity and LOS issues while keeping readmissions low. The Manager supervises the case management staff in planning implementing, and re-evaluating the psycho-social and discharge planning needs of neonatal, pediatric, adolescent, adult, and geriatric patients. The Manager leads interdisciplinary teams in process improvement activities to improve capacity, LOS, readmission and the patient experience. The Manager actively participates in assuring outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.
Qualifications
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:

Identifies and supports campus goals for capacity management through achieving length of stay goals; identification of Avoidable Delays at department approved levels, implementation of readmission reduction strategies, improvement of patient transition of care processes and other Case Management initiatives.
Assesses, plans, implements and evaluates departmental goals and objectives in cooperation with the staff.
Demonstrates expertise in patient/family assessment incorporating physical, psychosocial, environmental, spiritual, and financial factors to lead case management staff to develop sound discharge plans to assure proper patient placement across the healthcare continuum and to maximize hospital throughput and capacity.
Oversees with the Director of Case Management the operational and financial aspects of departmental operations
Applies knowledge of local, state, and federal programs to expedite discharge while being a good steward of healthcare resources.
Reviews the provision of the discharge plan to make sure the age and developmental needs of the patient population are taken into account, as well as patient choice.
Assists with ongoing campus-specific case management performance improvement program, ensuring a single standard of care, within individual campus requirements.
Manages staff performance by follow-up on accountabilities, individual meetings, counseling, and evaluation.
Participates in campus and system utilization review committees as appropriate identifying patients to bring to committee, meeting with physicians, and maintaining relationships with physician members.
Partners with Risk and Legal teams to address complex care coordination needs.
Partner with health care team to implement and sustain interdisciplinary rounds and patient and family care conferences.
Identifies, implements and evaluates departmental learning needs and facilitates systematic ongoing education according to department need.
Maintains successful relationships with hospital administration, management, physicians, patients and their families, and external providers as appropriate.
Performs other duties as assigned or directed to ensure the smooth operation of the department

KNOWLEDGE AND SKILLS REQUIRED:

KNOWLEDGE AND SKILLS PREFERRED:
Electronic Medical Record experience

EDUCATION AND EXPERIENCE REQUIRED:
BSW with a health care related Masters Degree (MSW or MHA) or RN with BSN (or related BA/BS)
3 years experience in discharge planning in an acute care hospital

EDUCATION AND EXPERIENCE PREFERRED:
LCSW or MSN or other related Masters Degree (i.e.: MBA)
Management and/or Supervisory experience in case management, or in a related CM field (i.e. LTAC)
Experience in overseeing programs that address capacity and LOS issues.
Experience in Readmission prevention strategies

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
If a nurse, must have current licensure as State of Florida Registered Nurse

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
Licensed Clinical Social Worker or CCM (Certified Case Manager) or ACM (Accredited Case Manager)
