The Community Health Navigator works in collaboration with the COVID-19 Response Initiative at Esperanza Center to provide case management services to immigrant households.She/he will manage a case load of individuals and their families in Baltimore City who have been impacted by COVID-19. Specific duties include assessing each clients health, social, and financial needs, connecting clients and their families to appropriate services such as food delivery, cash assistance, isolation hotel, SNAP benefits, behavioral health services, or other services and resources as needed, eligible, and available. She/he will also provide education on COVID-19 and isolation protocols.



The Community Health Navigator is responsible for:



  • Provide dedicated case management for a period of 8-10 weeks to individuals who receive a positive COVID-19 test result, and their families
  • Assess health, social, and financial needs of clients and their households in order to make service referrals and resource connections

  • Provide guidance regarding isolation for clients diagnosed with COVID-19

  • Identifies and develops relationships with social service agencies serving the Baltimore City immigrant community

  • Prepares and delivers home safety kits to impacted households to include face masks, hand sanitizer, and other items deemed necessary.

  • Documents all client encounters in Service Point, and other HIPAA-secured platforms as required

  • Maintains professional/personal boundaries with clients, keeping the position Supervisor apprised of all situations of possible conflict, and maintain strict confidentiality of all patient information.

  • Prepares and processes invoices, check requests, field expense vouchers and other documents related to program expenses (travel, food, material purchases, etc. ) as appropriate.

This position requires:



  • High School Diploma.
  • Life/community/work experience or knowledge regarding the Latino Community and infectious diseases (Covid-19) preferred.

  • One Year experience working with a culturally diverse population required.

  • Working knowledge of community resources and ability to connect families to community services.

  • Bi-Lingual in English and Spanish required.

  • Ability to learn and master database software.

  • Requires the ability to utilize computer systems and software necessary to perform position functions. Basic Windows PC, web browsing (i.e. Chrome, Internet Explorer, etc.), and Microsoft Outlook skills required. Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Skype, and OneDrive desired.

  • Must have reliable transportation to visit multiple sites in Baltimore City. If driving on Agency business, must be at least 21 years of age and have valid drivers license issued by the state of residence, with no more than 3 points.
