The Developmental Case Manager (DCM) will work in conjunction with staff members of the CATCH team and Pediatrics Department, as well as providers, to support and offer ongoing assistance to families of children with Neurodevelopmental Disabilities (NDD), including Autism Spectrum Disorder (ASD). The Developmental Case Manager will attend Developmental Clinic in person to provide direct support to patients and families, as well as phone and in person follow up outreach to families and patients. They will be responsible for care coordination, providing connections to community based supports, therapies and interventions, meeting with families, and providing home visits as appropriate.


  • Provide direct support to families of patients with NDD and ASD through individualized, tailored resources and supports to navigate a child's autism diagnosis.

  • Maintain consistent and ongoing follow-up with families to assure appropriate follow through on tasks, and to troubleshoot barriers when appropriate. Utilize exceptional problem solving skills to assist families in overcoming barriers to a childs service needs.

  • Execute referrals to state agencies, community-based programs and service providers. Completes paperwork on behalf of families as needed.

  • Identify NDD or ASD-specific community supports- such as playgroups, recreational activities, social skills groups, camps.

  • Integrate culturally informed practices in work with diverse patient population. Utilize interpreter services as needed.

  • Routinely support children, families and clinicians in-person during patient appointments.

  • Coordinate appointments at EBNHC and specialists. Will have the ability to schedule appointments with primary care as well as developmental follow up visits.

  • Performs timely documentation in the electronic medical record (EPIC) in a standardized format.

  • Maintain excellent communication with health care clinicians, ancillary support staff and related service professionals to see that care needs are addressed in a timely manner.

  • Seek community collaborations and partnerships to strengthen supports available to families.

  • Utilize weekly supervision with CATCH NP to review patient referrals and identify action steps. Integrate constructive feedback.

  • Participate in additional responsibilities within the broader CATCH Program, including cross coverage.

Communication and Organization:

  • Maintain a current list of resources to share with providers and families- online, on SharePoint, in English and Spanish.

  • Manage iPads for use with Developmental Clinic.

  • Communicate with Care Navigators regarding ABA application status and help provide referral information.

  • Obtain release of information for all patients- school, ABA.

  • Serve as a resource for Care Navigators and AmeriCorps Volunteers.

Patient Education and Care Coordination:

  • Provide education and guidance around guardianship process.

  • Educate families on purpose of Early Intervention, and assist them to access EI services.

  • Outreach school staff regarding teacher feedback and obtaining copy of IEP.

  • Assist families and patients applying for DDS, SSI, PCA, and help complete applications.

  • Curate learning materials to provide to families and patients, for example, social stories, weighted vests, ABA at home videos.

  • Request and follow up on diaper, enteral and other DME prescriptions.

  • complete PT-1 application.

  • Connect patients with resources for concrete needs such as food stamp applications, food pantry information, referrals to patient resource center for help with housing instability, applications for cradles to crayons.

  • Help patients and families learn to access MyChart to facilitate communication with their providers.


  • Maintain excellent punctuality and attendance.

  • Promote a sense of team work through demonstration of self-direction and self-motivation. Solve problems independently or knows when to seek consultation.

  • Displays outstanding customer service skills when interacting with all EBNHC customers according to the ACE model.

  • Adheres to all EBNHC and departmental polices and procedures.

  • Participate in required departmental activities and meetings.

Bachelors Degree preferred. (*An equivalent combination of education and experience, which provides proficiency in the areas of responsibility, may be substituted for the college degree).

1-2 year previous work experience required. Experience may be waived for candidates with a Bachelors Degree or above)

Experience in maternal and child health preferred. Experience working in special education, with special needs children, or with people with Autism preferred.
