Responsibilities

The Venice Family Clinic is a community health center whose
mission is To provide quality primary health care to people in need. Venice
Family Clinic cares for over 28,000 low-income patients in over 120,000 visits
annually who might otherwise go without critically needed health care services.
Venice Family Clinic has eleven clinical sites with over 350 staff, 1,700
volunteers, and a budget of $50 million. We provide high quality, comprehensive
primary and specialty health care, dental, mental health, substance use
treatment and child development services to underserved individuals and
families on the Westside of Los Angeles.

As our Health Homes Case Manager, you will provide case management
services to medically complex high-risk individuals, including the provision of
intervention and support to clients and close collaboration with the
multidisciplinary Primary Care team. You will contact patients for outreach and engagement and complete
Health Homes Program enrollment per protocols and will conduct comprehensive client intakes and
assessments, including demographic information and psychosocial history. In this role, you will also then develop a Health
Action Plan with each client and update as needed and provide information
regarding referrals and assure that linkage is completed. You will collaborate with assorted social service agencies
with the aim of building and maintaining a contact network of local community
partners to expedite linkages for clients.

Qualifications

  • Full fluency bilingual English/Spanish skills required.
  • Ability to work effectively with people of
    diverse cultural and educational background
  • Ability to discuss sensitive matters in a non-judgmental manner and to
    respect confidentiality
  • Skill in
    maintaining cooperative working relationships with staff members and
    volunteers
  • Skill in managing multiple
    deadlines and demands .
  • Ability to work independently and as part of a
    multidisciplinary team
  • Ability to
    follow through on assignments
  • Current
    CA drivers license, own transportation, auto insurance
  • BA or BS in Nursing, Social Work, Counseling,
    Psychology, OR equivalent care coordination experience OR a license as a
    vocational nurse (LVN) preferred
  • Skill in creating flyers and other program-promoting documents.
