Central Ohio Primary Care Physicians is seeking a Manager of Acute Care Services. The Manager of Acute Care Services is responsible for the day-to-day operations of programs within the Acute Care Service (ACS) portfolio such as the Post-Acute Program, Comprehensive Home and Palliative Care, Memory Assessment Center and Extensive Care Center. This role will support the optimization, growth, and efficacy of the ACS programs in collaboration with clinical and administrative leaders across COPC and partner organizations.



Duties/Responsibilities:



  • Provide day-to-day clinical operations support to ACS clinicians and staff including resolution of issues and questions, use judgement in escalating issues when appropriate.

  • Participates or leads the development, implementation, and optimization of clinical processes in partnership with clinicians and staff, focusing on the goals of successful patient outcomes, collaboration, and stewardship.

  • Support the achievement of program specific goals through the use of data, stakeholder and patient feedback, and influence with internal and external partners.

  • Accountable for all aspects of people management including hiring and onboarding, performance reviews, payroll and time-keeping, scheduling, and the support and professional growth of staff.

  • Acts as backup when employee is out on PTO or for unplanned absences.

  • Serve as a liaison between ACS programs and internal teams to ensure collaborative relationships and support the awareness and growth of the ACS portfolio.

  • Support program measurement and reporting by monitoring error reports, providing documentation education, and recommending documentation process changes to improve quality and efficiency.

  • Assume responsibility for practice management and onboarding activities for Acute Care Services providers.

  • Ensure continued growth and clinical education/skill development of team members; develop growth plans and assist in the curation of clinical education, experiences, and resources.

  • Establish meeting agendas in advance of meetings; coordinate attendees and their contributions/deliverables for each meeting; manage meeting times and topics; at the end of each meeting summarize important decisions made, what action items are due and by whom, when the next meeting will occur.

Requirements:



  • Licensed Social Worker or Registered Nurse or in the state of Ohio.

  • Master's in nursing, social work, healthcare administration, business management, project management or a related field preferred

  • 3-5 years of experience in a healthcare, program management or process improvement related organization.

  • 1-3 years of experience in people management with accountable care organizations or at-risk arrangements.

  • Knowledge of best practices regarding reporting, workflows, policies, procedures, systems, and culture in the healthcare environment.

  • Strong organizational skills and customer service orientation.

  • High functioning team skills, including active listening and rapport building.

  • Effective written & verbal communication.

  • Ability to identify, document, resolve and/or escalate issues.

  • Ability to communicate with stakeholders at all levels. With a strong ability to communicate effectively with executive level management.

  • Ability to learn new technologies and business concepts.

  • Self-disciplined, energetic, passionate, and innovative.
