In response to the COVID-19 pandemic, San Francisco is working to provide urgent care and referral to primary care for recently homeless adults presently living in shelter- in-place facilities (tourist hotels, safe sleeping sites, etc.) The Whole Person Integrated care team is working as a multi-disciplinary group to assess, treat and refer up to 2500 adults and families that have recently been placed in these facilities.





This is a temporary, grant-funded, full time, benefited position. Employment is provided by Heluna Health.





ESSENTIAL FUNCTIONS



Under the direction of the Medical Director for Whole Person Integrated Care Shelter Health, the Shelter in Place medical providers serves to provide direct care to this diverse group of frail homeless adults who have recently been moved off the street and out of shelter. The SIP Medical provider performs the following essential functions:





  • Provides direct urgent and episodic medical care to adults and families.

  • Collaborates with behavioral health team to provide care for individuals with mental health and substance use disorders

  • Prescribes buprenorphine to individuals with opioid use disorder using WPIC low barrier MAT protocols

  • Reviews and assesses quality improvement to determine appropriate interventions, and evaluate effectiveness of response efforts

  • Provides consultation to advanced practice clinicians, nurses and health workers to improve quality care and report to the medical director as to quality of service.

  • Develops and tracks metrics to measure progress of service delivered

  • Provides situational updates to co-lead and team members

  • Provides input and approval of standard work and protocols for team

  • Provides leadership and assistance to refer to primary care for the guests of the facilities

JOB QUALIFICATIONS



  • Experience in primary care and public health for adults and families.

  • Experience in program management, staff supervision and leading a team

  • Experience in quality improvement methods

  • Excellent written and oral communication skills

  • Sensitivity to and experience working with diverse racial/ethnic communities

  • Experience working with congregate living settings

  • Experience working in a government agency at the local, state, or national level

  • Comfortable learning and working with different computer applications and health records

  • Experience working with people experiencing homelessness with multiple problems

  • Experience and training in Addiction Medicine

Education/Experience



  • Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education or American Osteopathic Association in Emergency Medicine and Board Eligible in an appropriate medical specialty.

Certificates/Licenses/Clearances



  • Possession of a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California and possession of a valid DEA license.

  • Strong written and oral communication skills

  • Experience working in multi-disciplinary collaboratives

  • Experience working in a government agency at the local, state, or national level.

  • Experience in public health preparedness and emergency response
