Job Summary
Under the direction of the Patient Safety Officer, assists in the development, implementation, and coordination of patient safety programs within the hospital. Responsible for identifying methods for reduction of medical/health care errors and other factors that contribute to unintended adverse outcomes, and facilitating integration of literature-based recommendations for continued enhancement of patient safety program.




Job Specific Duties



  • Under the direction of the Patient Safety Officer, assists in the development, implementation, and coordination of patient safety programs within the hospital.

  • Identifies methods for reduction of medical/health care errors and other factors that contribute to unintended adverse outcomes, and facilitates integration of literature-based recommendations.

  • Coordinates the Patient Safety Climate survey within the hospital.

  • Rounds through the medical facility, gathering information, and interviewing different employees to understand where safety can be improved.

  • Evaluates patient-safety knowledge of employees and recommends changes to training programs.

  • Coordinates the activities of the patient safety committees.

  • Assists in the development of and implementation of patient safety improvement projects within the hospital while working in conjunction with physicians, directors and clinical staff.

  • Supports the collection, analysis and reporting of patient safety data to meet national, state, regional, and internal measures.

  • Recommends changes, policies, or programs that could prevent future errors.

  • Collaborates with Risk, Quality, & Patient Safety partners in gathering data detailing patient-related errors/near-misses, summary reporting, data analysis, response planning & implementation.

  • Follows up on all events through apparent cause analysis and root cause analyses.

  • Conducts periodic Common Cause Analyses of individual and system failure mode data within the hospital.

  • Educates and coaches hospital staff in conducting Apparent Cause Analyses on precursor and potentially near miss events.

  • Educates and reinforces hospital staff in high reliability behavioral based expectations.

  • Gives presentations and training programs meant to increase awareness of patient safety initiatives.

  • Conducts Failure Modes and Effects Analyses on patient safety processes.


Minimum Job Requirements



  • Bachelor's Degree in nursing, pharmacy, or related clinical area

  • 5 years in healthcare setting

  • 2-4 years experience in quality, risk, or patient safety activities

Knowledge, Skills, and Abilities



  • CPHRM, CPHQ, and/or CPSO Certification preferred

  • Experience in healthcare/clinical education

  • Proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.

  • Leadership capabilities

  • Effective written, verbal and presentation skills

  • Proficient in event analysis methodologies

  • Knowledge of high reliability methodology

  • Able to maintain confidentiality of sensitive information.

  • Able to prioritize to successfully meet deadlines and handle multiple concurrent tasks.

  • Able to work effectively in a team environment.
