Provide family driven planning and meeting facilitation for families with children from prenatal through age 17 who are affected by parental Substance Use Disorders with the goal to help families develop a natural support system that will sustain their success into the future.

Key Responsibilities:

1. Complete Wraparound Process with approximately 10-12 families/year for 4 years, according to the NH Wraparound Model
2. Works with families to identify their strengths and needs, set a family vision/goal, develop their plan and then monitor the outcomes of the plan
3. Leads a Wraparound care team for each family, seeking to understand and support families with long term and short term needs.
4. Works closely with the Dartmouth Trauma Interventions Research Center and Center for Program Design and Evaluation at Dartmouth to instill a rigorous research study of the Wraparound process.
5. Works with Wraparound Coaches to learn and practice competencies of the Model with a goal of certification in one year.
6. Maintains training and consultation requirements.
7. Works in collaboration with family and clinical team to develop and monitor Wraparound care and crisis plans.
8. Work in collaboration with the Community Health Worker community at DHMC.
9. Develops and maintains collaborative relationships with child, family, and relevant community partners, including pre and post-natal services, addiction and recovery services, early childhood services, schools, and child welfare.
10. Provides parent-child relationship support and/or relevant psycho education, developmental guidance.
11. Assists child/family in obtaining and maintaining resources and services in the community.
12. Performs other duties as required or assigned.

Minimum Qualifications:
Bachelors level or higher degree
At least 3 years work experience in local (Sullivan/Grafton county NH) child/family serving system (can include internships) such as child protection, substance use, or criminal justice experience preferred.
Successful criminal check, No history of abuse, neglect, or exploitation.
Must have reliable transportation and valid drivers license and auto insurance.
Must possess excellent oral and written communication skills and be able to solve problems independently.
Be a collaborative partner yet seek out counsel when needed
Use trauma-informed, destigmatized language, conceptualization, and interactions with families and collaborators
Must have necessary empathic and relational skills to meet families where they are, provide unconditional positive regard, and believe in the dignity and resiliency of parents and caregivers who experience trauma and/or significant substance use while parenting.
